Registration is open. Register by December 1st, and receive a $25 discount, and additional payment options.

REQUIRED FOR REGISTRATION (GRADES K-12)
- A valid USA Lacrosse number.
- Medical information for your player(s) including: Doctor's contact info, insurance provider and policy information.
- All required waivers and releases for all grades will be signed online.

All Players must be registered with the US Lacrosse League on the USA Lacrosse website
CANCELLATION POLICY FOR ALL REGISTRATIONS
Tulsa Alliance Lacrosse will refund 100% of registration costs (excluding US Lacrosse Membership fees) prior to February, 15. All refund requests must be made in writing and sent via e-mail to Mary Jo Kimbro (emmittmj@outlook.com). It is the responsibility of the parent/guardian to request a refund. After February 15 NO REFUNDS WILL BE GIVEN, even for injuries or grade issues during the season.
